Good Faith Estimate (GFE)

Federal law requires that you receive a Good Faith Estimate (GFE) for out-of-pocket services. GFE explains the reasonably expected out-of-pocket costs for the requested services to address your mental health care needs. The estimate is based on the information known to the clinician at the time of the given estimate. The GFE does not include any unknown or unexpected costs that may arise during treatment.

The total cost for the psychological evaluation will be discussed at the onset and may vary depending on the nature of the clinical issue being assessed. Sometimes, the approach for the evaluation process may need to change by administering additional psychological instruments, which can possibly increase the cost of the evaluation. The clinician will not move forward with the changes until they are discussed and agreed upon. You are not obligated to agree to the additional testing costs. However, if you decline to move forward with the additional testing and costs, this may impact the clinician’s ability to provide a thorough evaluation. You are free to terminate services at any time. However, you will be responsible for any unpaid balance of services received.

Payment

  • At each session, payment is due unless another payment plan has been established. Forms of payment accepted are cash, credit, debit, and HSA/FSA cards. Clients may request an itemized receipt or payment (super bill) for services received, which will be provided after the evaluation has been completed. This receipt may be submitted to the insurance by the client. It is not guaranteed or promised that the client will be reimbursed for services.
  • Accepted forms of payment are cash, credit, debit, and HSA/FSA cards.
